The type of personal information Enlighten your Health collects and what I do with it
In order to give professional reflexology treatments, I will need to ask for and keep information about your health. I will only use this for informing reflexology treatments and any advice I give as a result of your treatment. 
 
The information to be held is:
· Your contact details
· Medical history and other health-related information (which I will take from you at first consultation)
· Treatment details and related notes (which I will take after each treatment)
 
I will NOT share your information with anyone else, (other than as required for legal process), without explaining why it is necessary and getting your explicit consent.

The purpose of processing client data
For the purpose of providing professional treatments, Enlighten your Health will need to gather and retain potentially

sensitive information about your health. I will only gather information that is relevant and necessary for your treatment. During your initial consultation I will make notes, which may include details concerning medication, treatment, and any other issues affecting your health. I make such notes to ensure I can adapt the treatment to best suit you, and to make accurate and precise recommendations concerning aspects of holistic health and wellbeing, which I will offer to you. I take basic contact details and information via my website to allow me to contact you and handle bookings. Any data provided is held securely, and privately. In order to be able to process your personal data, it is a condition of any treatment that you give your explicit consent to allow Enlighten your Health to document and process your personal medical data. Contact details, such as your email address or phone number, may be used to remind you of future appointments, and provide reports or other information concerning your treatment.                                                                                                        
 
Lawful Basis for holding and using client information

Under the UK General Data Protection Regulation (UK GDPR), the lawful basis Enlighten your Health relies on for processing this information are:
 

• Your consent, you are able to remove your consent at any time. You can do this by contacting enlightenyourhealth2015@gmail.com

 

• I have a legal obligation

1.1. ‘Claims occurring’ insurance: (records to be kept for 7 years after last treatment) 
1.2. Law regarding children’s records (records to be kept until the child is 25 or if 17 when treated, then 26) 
 

• I have a legitimate interest, i.e. my requirement to retain the information in order to provide you with the best possible treatment options and advice

 

• I hold special category data, i.e. health related information. The additional condition under which I hold and use this information is for me to fulfil my role as a health care practitioner bound under the AoR Confidentiality as defined in the AoR Code of Practice and Ethics. 

 
How long I retain your information for
 
Enlighten your Health will process personal data during the duration of any treatment and will continue to store only the personal data needed for 7 years after the contract has expired, to meet any legal obligations. After 7 years all personal data will be deleted or safely disposed of unless basic information needs to be retained by me to meet my future obligations to you. Any records concerning minors who have received a treatment will be retained until the child has reached the age of 25.
 
Protecting your personal data
 
I am committed to ensuring that your personal data is secure. In order to prevent unauthorised access or disclosure, I have put in place appropriate technical, physical and managerial procedures to safeguard and secure the information I collect from you.
 
I will contact you using the contact preferences you give me in relation to: 
 

• Appointment times 

• Reflexology information or information related to your health 

• Special offers, updates and promotions (you may unsubscribe from this at any time)

Under data protection law, you have rights including:

Your right to be informed – You have the right to know how your information will be held and used
Your right of access - You have the right to ask for copies of your personal information, so you know what is held about you and you can verify it.

Therapist’s rights

If you don’t agree to me keeping records of information about you and your treatments, or if you don’t allow me to use the information in the way I need to for treatments, I may not be able to treat you.
 
I have to keep your records of treatment for a certain period as described above, which may mean that even if you ask me to erase any details about you, I might have to keep these details until after that period has passed.
 
I can move my records between my computers and IT systems, as long as your details are protected from being seen by others, without your permission.
